Eligibility:  Currently registered USMS swimmers, 18 years and older.  Unregistered swimmers must submit a 2011 registration form and fee with this form.

Date:  Sunday, June 12, 2011
            Warm-ups:  7:30 AM

Location:  Osborn Aquatic Center, 1940 NW Highland Drive, Corvallis, Oregon 97330.

50 Meters - 6 lanes competition.  Continuous warm-up/warm-down in lane 1.

Directions to Pool:  Take I-5 to Exit 228 (Highway 34). Go west on Hwy 34, following signs to Corvallis.  Hwy 34 turns left, but keep going straight over the Willamette River.  Continue straight through four stoplights.  Turn right at the 5th light onto 9th Street.  Take 9th Street to Circle Blvd and turn left.  Go one block, turn left at Highland Drive, and the pool is on your right.

Entry Deadline:  Online entries are preferred.  Online registration must be completed no later than 11:59 PM PDT, Wednesday, June 3, 2011.  Paper entries must be postmarked no later than June 3, 2011.

Age groups: 18-24, 25-29, 30-34, etc . up to 100+. Relay age groups: 72-99, 100-119, 120-159, 160-199, 200-239, 240-279, 280-319 and 320-359 etc. Your competition age is the age you will be as of Dec 31, 2011. Enter relays at the meet. All events will be seeded SLOW TO FAST.

Pentathlon Entries:  Swimmers can choose from three separate pentathlon events:
1)  Sprint Pentathlon:  50s of each stroke plus the 200 IM - 400 meters total.
2)  Mid-Distance Pentathlon:  100s of each stroke plus 200 IM - 600 meters total.
3)  Long Distance Pentathlon:  200s of each stroke plus 400 IM - 1200 meters total.

Disqualifications and Scoring:  A disqualification in any one event of the pentathlon will disqualify a swimmer from the pentathlon, but will not prevent the swimmer from competing in the other individual events.  Scoring shall be based upon the cumulative times of the five pentathlon events.

Entry Limits:  You may enter a maximum of 5 individual events, plus relays. The 400 IM will be limited to those entering the full distance pentathlon. The 400 medley relays will be limited to no more than one heat of mixed, men and women relays combined. Entries for the 400 medley relays will close at the beginning of the 100 Free (Event #9).  The 400 medley relays will be permitted contingent upon projected timeline.
